Distinguish Type 1 From Type 2 Diabetes in Obese Youth

STEAMBOAT SPRINGS, COLO. – New-onset type 1 diabetes in an obese youth
cannot reliably be distinguished from pediatric type 2 diabetes on
clinical grounds in this era of epidemic obesity.

"The only way to distinguish obese type 1 diabetes from type 2
diabetes is to measure diabetes autoantibodies. And those autoantibody
panels are commercially available now. Signs and symptoms, diabetic
ketoacidosis, family history – they don’t really help you. We get an
autoantibody panel routinely in obese kids above age 10 presenting
with new-onset diabetes," said Dr. Charlotte M. Boney, chief of the
division of pediatric endocrinology and metabolism at Hasbro
Children’s Hospital in Providence, R.I.

Diabetic ketoacidosis is widely thought of as incompatible with type 2
diabetes. Not true. Close to 20% of youth with type 2 diabetes present
with DKA. Similarly, while a history of recent weight loss is
considered a classic presenting symptom of type 1 diabetes, it’s also
present in about one-quarter of young people presenting with type 2
diabetes, Dr. Boney noted.

The presence of pancreatic autoantibodies spells type 1 diabetes
metabolically, even if the patient appears phenotypically to have type
2 disease.

"Some pediatric endocrinologists call this ‘type one-and-a-half’
diabetes. No, no, no. Let’s not make things any weirder than they
already are. They have autoimmune diabetes, which is clearly type 1
diabetes. It just happens to be a little more complicated in them
because they also have the morbidity of obesity," she explained at the
meeting on practical pediatrics sponsored by the American Academy of
Pediatrics.

The obesity epidemic has muddied the diagnostic waters, because now
20%-30% of patients with new-onset type 1 diabetes are obese, as is a
similar proportion of the general pediatric population. At the same
time, the obesity epidemic has led to an increase in type 2 diabetes.

But it’s important to bear in mind that most youths with new-onset
diabetes still have type 1 disease, she said.

In the landmark prospective The SEARCH For Diabetes In Youth study,
nearly all children who presented under age 10 years had type 1
diabetes. Among 10- to 19-year-olds, the proportion with type 2
disease was 15% among whites, but considerably greater among racial
minorities: 58% among African Americans, 46% in Hispanics, 70% in
Asian/Pacific Islanders, and 86% among Native Americans (JAMA
2007;297:2716-24).

In the ongoing, multicenter, National Institutes of Health–sponsored
Treatment Options for Type 2 Diabetes in Adolescents and Youth (TODAY)
study, which enrolled 1,206 subjects with a presumptive diagnosis of
type 2 diabetes, 9.8% proved to be positive for GAD-65and/or
insulinoma-associated protein 2 autoantibodies. They had to be
excluded from participation in the treatment phase (Diabetes Care
2010;33:1970-5).

As a practical approach to the initial therapy of young patients with
new-onset diabetes, Dr. Boney urged that those with DKA and ketosis
should be started on intravenous fluids and insulin, regardless of
their age and body habitus. If they are over age 10 and obese,
however, pancreatic autoimmunity should be ruled out before
transitioning to long-term therapy. For autoantibody-negative patients
whose clinical picture is consistent with type 2 diabetes, the
treatment is metformin, the only Food and Drug Administration–approved
therapy for children. Extensive experience shows that it’s a very safe
drug, she said.

The TODAY trial is designed to determine whether the best treatment
for type 2 diabetes in youth is metformin alone, metformin plus
rosiglitazone, or metformin and an intensive lifestyle intervention
aimed at achieving a 7%-10% weight loss.

The use of metformin to try to prevent diabetes in obese children with
insulin resistance and the metabolic syndrome is the subject of large
ongoing clinical trials. Until the results come in, Dr. Boney said she
sees no role for off-label prescribing of metformin, given that weight
loss and exercise are quite effective in improving insulin
sensitivity.

Maturity Onset Diabetes of the Young, or MODY, is worth considering in
white youth who are pancreatic autoantibody–negative and have a strong
history of parental non–type 1 diabetes. MODY is a single-gene
disorder that causes diabetes and is inherited from a parent.

"There are a lot of experts in the MODY field that think we’re grossly
underdiagnosing monogenic diabetes," said Dr. Boney.

The treatment for MODY is not insulin or metformin, but rather oral
sulfonylureas, although those agents are not FDA-approved for use in
children, she observed.

$5 Trillion and Change

Obama's four years have seen the four highest deficits since 1946.

The political strategy behind Obamanomics was always simple: Call for
"stimulus" to rescue the economy, run up the debt with the biggest
spending blitz in 60 years, and then when the deficit explodes call
for higher taxes. The Congressional Budget Office annual review
released yesterday shows this is all on track.

CBO reports that annual spending over the Obama era has climbed to a
projected $3.6 trillion this fiscal year from $2.98 trillion in fiscal
2008, or more than 20%. The government spending burden has averaged
24% of GDP, up from an average of about 20%. This doesn't include the
$2 trillion tab for ObamaCare.

All of this has increased the federal debt by about $5 trillion in a
mere four years. Thanks to higher revenues, the federal deficit will
decline to $1.08 trillion in 2012, or 7% of GDP. But that is still the
highest deficit since 1946—except for the previous three years. In
other words, the four years of the Obama's Presidency will mark the
four highest years in spending and deficits as a share of the economy
since Harry Truman sat in the Oval Office.

We pause from this gloom for some good news: Despite the abuse they've
taken, House Republicans have made some fiscal progress. CBO estimates
that overall federal spending in 2012 will grow by only $3 billion, or
less than 1%, which compares with double digit increases during the
Obama-Pelosi years. Republicans have also tried to reform
entitlements, but Democrats wanted a $1 trillion tax hike ransom for
even modest cuts, which was wisely rejected.

The other part of the fiscal story is that revenues have been in the
tank for five years. In 2012 revenues will hit $2.52 trillion down
from $2.57 trillion in 2007. Revenues are still only 16.3% of GDP,
about two percentage points below the norm.

The drought has two main causes. First, the anemic recovery in jobs
and investment isn't spinning off enough new output (1.7% growth last
year) to boost tax receipts anywhere near their historic level.

Second, a series of non-stimulative tax cuts—tax rebates in 2008 and
2009, and payroll tax holidays in 2011 and this year—have depleted the
Treasury with little economic benefit. These tax cuts don't change the
incentive at the margin to work or invest, and they thus have little
feedback effect in revenues from faster growth.

The most amusing part of the CBO's report is its projection that the
deficit will fall to $269 billion by 2015, or a mere 1.5% of GDP, if
current law holds. But this is a fiscal fantasy because current law
never holds.

CBO predicts, for example, that all the Bush tax cuts will go away
next year. The Alternative Minimum Tax will supposedly be allowed to
hit 30 million tax filers (up from four million now) with an income as
low as $75,000 a year. Under those assumptions total federal revenues
rise by $1 trillion over the next three years, $1.5 trillion over five
years, and $3.6 trillion over 10 years. You can't get anywhere near
that level of revenues without a much bigger tax increase on the rich

Even the Keynesians who run CBO concede that the 2013 tax hike—on
capital gains, dividends, estates and small business—would knock
economic growth down to 1% next year and raise unemployment to 9.1%
(from 8.5%). That means about 750,000 more jobless Americans. You
can't have such a lousy economy and cut the deficit in half.

CBO also indulges in the fantasy that discretionary spending will fall
by nearly $2 trillion over the next decade—with almost all the cuts
after 2015. About $1.25 trillion of those cuts come from the automatic
across-the-board reductions that Congress and Mr. Obama agreed to last
year. But wait. More than half of those cuts will come from the
military budget and even Defense Secretary Leon Panetta has said these
reductions could be "devastating" to national security.

To sum it all up, CBO's facts plainly show that Mr. Obama has the
worst fiscal record of any President in modern times. No one else is
even close.

Abstract
The mucus layer coating the gastrointestinal tract is the front line
of innate host defense, largely because of the secretory products of
intestinal goblet cells. Goblet cells synthesize secretory mucin
glycoproteins (MUC2) and bioactive molecules such as epithelial
membrane-bound mucins (MUC1, MUC3, MUC17), trefoil factor peptides
(TFF), resistin-like molecule beta (RELMbeta), and Fc-gamma binding
protein (Fcgbp). The MUC2 mucin protein forms trimers by disulfide
bonding in cysteine-rich amino terminal von Willebrand factor (vWF)
domains, coupled with crosslinking provided by TFF and Fcgbp proteins
with MUC2 vWF domains, resulting in a highly viscous extracellular
layer. Colonization by commensal intestinal microbiota is limited to
an outer "loose" mucus layer, and interacts with the diverse
oligosaccharides of mucin glycoproteins, whereas an "inner" adherent
mucus layer is largely devoid of bacteria. Defective mucus layers
resulting from lack of MUC2 mucin, mutated Muc2 mucin vWF domains, or
from deletion of core mucin glycosyltransferase enzymes in mice result
in increased bacterial adhesion to the surface epithelium, increased
intestinal permeability, and enhanced susceptibility to colitis caused
by dextran sodium sulfate. Changes in mucin gene expression and mucin
glycan structures occur in cancers of the intestine, contributing to
diverse biologic properties involved in the development and
progression of cancer. Further research is needed on identification
and functional significance of various components of mucus layers and
the complex interactions among mucus layers, microbiota, epithelial
cells, and the underlying innate and adaptive immunity. Further
elucidation of the regulatory mechanisms involved in mucin changes in
cancer and inflammation may lead to the development of novel
therapeutic approaches.

Abstract
Several studies have stressed the importance of the microbiota in the
maintenance of the gastrointestinal epithelium. Administration of
probiotic bacteria, supplements composed of microbiota constituents,
was previously shown to diminish symptoms in patients suffering from
inflammatory bowel diseases. This raises the possibility that
probiotics may play an active role in enhancing the intestinal barrier
at the mucosal surface. In this study, we investigated whether the
clinically tested VSL#3 probiotic formula and/or its secreted
components can augment the protective mucus layer in vivo and in
vitro. For in vivo studies, Wistar rats were orally administered the
probiotic mixture VSL#3 on a daily basis for seven days. After
treatment, basal luminal mucin content increased by 60%. In addition,
we exposed isolated rat colonic loops to the VSL#3 probiotic formula,
which significantly stimulated colonic mucin (MUC) secretion and MUC2
gene expression; however, MUC1 and MUC3 gene expression were only
slightly elevated. The effect of the VSL#3 mucin secretagogue was also
tested in vitro by use of LS 174T colonic epithelial cells. In
contrast to the animal studies, cultured cells incubated with VSL#3
bacteria did not exhibit increased mucin secretion. However, the
bacterial secreted products contained in the conditioned media
stimulated a remarkable mucin secretion effect. Among the three
bacterial groups (Lactobacilli, Bifidobacteria, and Streptococci)
contained in VSL#3, the Lactobacillus species were the strongest
potentiator of mucin secretion in vitro. A preliminary
characterization of the putative mucin secretagogue suggested that it
was a heat-resistant soluble compound, which is not sensitive to
protease and DNase treatment. These findings contribute to a better
understanding of the complex and beneficial interaction between
colonic epithelial cells and intestinal bacteria.

The human gastrointestinal (GI) system is populated by thousands of
billions of beneficial bacteria that act as a protective barrier
throughout the GI system. These bacteria perform a variety of

Irritable Bowel Syndrome (IBS) is a common condition that negatively
impacts the quality of life for many individuals. The exact etiology
of this disorder is largely unknown; however, emerging studies suggest
that the gut microbiota is a contributing factor. Several clinical
trials show that probiotics, such as VSL#3, can have a favorable
effect on IBS. This double-blind, randomized placebo-controlled study
has been conducted in diarrhea-predominant IBS subjects in order to
investigate the effect of VSL#3 on the fecal microbiota. The bacterial
composition of the fecal microbiota was investigated using high-
throughput microarray technology to detect 16S RNA. Twenty four
subjects were randomized to receive VSL#3 or placebo for 8 weeks. IBS
symptoms were monitored using GSRS and quality of life questionnaires.
A favorable change in Satiety subscale was noted in the VSL #3 groups.
However, the consumption of the probiotic did not change the gut
microbiota. There were no adverse events or any safety concerns
encountered during this study. To summarize, the use of VSL#3 in this
pilot study was safe and showed improvement in specific GSRS-IBS
scores in diarrhea-predominant IBS subjects. The gut microbiota was
not affected by VSL#3 consumption suggesting that the mechanism of
action is not directly linked to the microbiota.

Could a probiotic be used to treat inflammatory bowel disease?
Scientists have been unclear for some time about how most probiotics
work. A new study has found a scientific 'design' for a probiotic
that
could be used to treat inflammatory bowel disease (IBD), such as
Crohn's disease.

The research by academics at the University of Bristol's School of
Veterinary Sciences and the School of Clinical Medicine is published
online in the journal PLoS ONE.

Most probiotics on the market, such as Lactobacillus and
Bifidobacterium, are lactic acid bacteria. Although probiotics have
been shown to successfully maintain remission in IBD, evidence of
their effectiveness in active disease is rare. The researchers have
found that this is because an increase in iron levels, which happens
during active IBD, inhibits the growth of probiotic bacteria,
including Lactobacillus.

Iron levels increase in the intestine during inflammation, bleeding,
during stress and when people are taking iron supplements. Iron is
critically important to the growth of most species of bacteria,
including pathogens, and its availability is what restricts their
growth. It is well known that pathogens increase growth rate by up to
8,000 times when exposed to increased levels of iron. Lactic acid
bacteria are unusual as they have evolved not to require iron, and so
do not increase growth rate when exposed to it.

Dr Tristan Cogan, Research Fellow in the School of Veterinary
Sciences, said: "When we started our study we considered the
properties that a probiotic would need to treat IBD. Most
importantly,
it would need to be able to survive and grow in the presence of high
levels of iron and to reduce inflammation. We then worked out how to
test bacteria to see whether it had these properties.

"The difficult step was finding a bacterium that had all of the
properties that we wanted. Now we have found something that looks
like
it should work, our next step will be to test the probiotic in
clinical trials."

The researchers found that a range of probiotic strains of lactic
acid
bacteria cannot respond to increased iron levels. The study then
identified a strain, which the academics have patented, that can and
showed that it has probiotic and immunomodulatory properties, and can
reduce pro-inflammatory responses to pathogens associated with IBD.

The researchers suggest that iron response is the factor that
prevents
probiotics from being useful in treating disease, and that their
bacterium could be a novel probiotic treatment for active intestinal
disease.

BACKGROUND:
Iron is an essential cofactor in almost all biological systems. The
lactic acid bacteria (LAB), frequently employed as probiotics, are
unusual in having little or no requirement for iron. Iron in the human
body is sequestered by transferrins and lactoferrin, limiting
bacterial growth. An increase in the availability of iron in the
intestine by bleeding, surgery, or under stress leads to an increase
in the growth and virulence of many pathogens. Under these high iron
conditions, LAB are rapidly out-competed; for the levels of probiotic
bacteria to be maintained under high iron conditions they must be able
to respond by increasing growth rate to compete with the normal flora.
Despite this, iron-responsive genera are poorly characterised as
probiotics.

METHODOLOGY/PRINCIPAL FINDINGS:
Here, we show that a panel of probiotics are not able to respond to
increased iron availability, and identify an isolate of Streptococcus
thermophilus that can increase growth rate in response to increased
iron availability. The isolate of S. thermophilus selected was able to
reduce epithelial cell death as well as NF-κB signalling and IL-8
production triggered by pathogens. It was capable of crossing an
epithelial cell barrier in conjunction with E. coli and downregulating
Th1 and Th17 responses in primary human intestinal leukocytes.

CONCLUSIONS/SIGNIFICANCE:
We propose that an inability to compete with potential pathogens under
conditions of high iron availability such as stress and trauma may
contribute to the lack of efficacy of many LAB-based probiotics in
treating disease. Therefore, we offer an alternative paradigm which
considers that probiotics should be able to be competitive during
periods of intestinal bleeding, trauma or stress.

I have been doing a similar diet for the past 18 months with great
success (95% improvement in PPP symptoms). However it isn't exactly
the same. Despite what Pagano says I've had no issues with berries at
all. I also discovered potatoes are harmless to me. Dairy? Absolutely
poisonous. Gluten? Dunno, but I don't eat it any more. And so I
suggest expanding your horizons a bit. I've read the Pagano book and
found some of his suggestions to be strange and poorly explained,
albeit I do recommend his book overall.

While on Pagano, he is spot on wrt linking colon health with
psoriasis. I had irritable bowel syndrome for years and so I knew my
colon was messed up. Diet and probiotics cleared that up 90%. I also
took light doses of grapefruit seed extract and olive leaf extract to
kill off unwanted pathogens. However recently I changed this to simply
do a heavier dose of olive leaf extract, in response to a (suspected)
mild lung infection following a cold. Well it zapped whatever was
bothering my lungs, and I also noticed my psoriasis improving. Google
"olive leaf extract" and you will see that it is safe. At any rate,
keeping your colon flora healthy is important. In his thread and
research paper Mikhail Inq discusses the link between colon issues and
psoriasis. I am a true believer in it.

First, there’s no new data in there—the LHC stopped colliding protons
back in November, and these latest results are just rehashes that
earlier run. In the case of the Compact Muon Solenoid (CMS),
physicists have been able to look at another possible kind of Higgs
decay, and that allows them to boost their Higgs signal from 2.5σ to
3.1σ. Taken together with data from the other detector, ATLAS, Higgs
overall signal now unofficially stands at about 4.3σ. In other words,
if statistics are to be believed, then this signal has about a 99.996
percent chance of being right.

It all sounds very convincing, but keep your hat on, because the fact
is that statistical coincidences happen every day. Over at Cosmic
Variance, Sean Carroll points out that there is a 3.8σ signal in the
Super Bowl coin toss. Does that mean that they’ve discovered a super-
partner to the bowl? No. (If you don’t get that joke, don’t worry, it
was written only as punishment for those who would).

Inhibiting PDE4 with rolipram reproduces all of the metabolic benefits
of resveratrol, including prevention of diet-induced obesity and an

also protect against and ameliorate the symptoms of metabolic diseases

Glucose tolerance? That is something that I have perhaps observed,
did not seem to get the rush from eating some hard candy, that I might
have expected.

I wonder what the half-life is for resveratrol, I suspect it's
somewhat long, on the order of days. Half-life of curumin seems much
shorter.

So maybe daily reseveratrol for weeks builds up. I've only tried it
for a few days at a time, so far.

derivative. To evaluate its suitability as a drug candidate, a
pharmacokinetic study was carried out in Sprague-Dawley rats with the
emphasis to identify the impact of aqueous solubility, dose
escalation, food, and repeated dosing on its oral bioavailability.
Upon single intravenous administration (5 mg/kg), RTE displayed
moderate clearance (35.5 ± 5.3 mL/min/kg) and a fairly long terminal
elimination half-life (511 ± 136 min); dose escalation (5-20 mg/kg)
did not cause nonlinear pharmacokinetics. When given orally in
suspension (60 mg/kg), RTE was poorly absorbed with negligible
bioavailability (< 1.5%), fasting further decreased its
bioavailability (<1%). However, when administered in a solution
formulated with randomly methylated-β-cyclodextrin (15 mg/kg), RTE was
rapidly absorbed with good bioavailability (46.5 ± 4.8%). Dose
escalation resulted in increased bioavailability (64.6 ± 8.0%) at the
dose of 60 mg/kg. Repeated RTE dosing (7 daily oral doses) did not
alter the clearance, terminal elimination half-life and
bioavailability. In summary, the aqueous solubility of RTE was a
barrier to oral absorption; repeated RTE administrations did not alter
its pharmacokinetic profiles; as RTE possessed appropriate
pharmacokinetic profiles, further investigation on RTE as a drug
