Note that these studies suggest that there is a high prevalence of
psoriasis among patients presenting with inflammatory bowel disease
and that psoriasis is an independent predictor for Crohn's disease in
women.
RALEIGH, N.C. -- Women with psoriasis had a four-fold increase in the
risk of Crohn's disease, according to data from two large cohort
studies.
The Crohn's risk held up in separate analyses of the two studies and
in a combined analysis of data from both cohorts.
Psoriasis did not increase the risk of ulcerative colitis in either of
the individual cohorts or in the combined analysis.
The findings reinforce those from smaller observational studies, and
from genomic analyses, that have suggested an association between
psoriasis and inflammatory bowel disease (IBD), as reported here at
the Society for Investigative Dermatology meeting.
Looking at the association from the opposite perspective, a second
study reported at SID showed a 13% prevalence of psoriasis among
patients with diagnosed IBD.
"Certainly, this is a preliminary study, but it does begin to make
sense because we are seeing patients with Crohn's disease and
psoriasis overlap," Abrar A. Qureshi, MD, of Harvard and Brigham and
Women's Hospital in Boston, told MedPage Today.
"The inflammatory pathways that have been identified at the genetic
level in genome-wide association studies makes sense as well because
there is overlap in the interleukin-12 and interleukin-23 pathways in
psoriasis and Crohn's disease," he said.
Overlap among psoriasis, IBD, and inflammatory or psoriatic arthritis
further adds to the concept that a common pathway is involved in all
of the diseases, said Qureshi, senior investigator in both of the
studies.
Several lines of evidence have supported an association between
psoriasis and IBD, reported Wenqing Li, PhD, also of Brigham and
Women's Hospital and Qureshi's co-author, during a presentation at
SID.
Particularly intriguing circumstantial evidence has come from genetic
and molecular studies. Both Crohn's disease and psoriasis are Th1-
mediated conditions. Ulcerative colitis is primarily mediated through
the Th2 pathway.
Susceptibility genes for psoriasis and Crohn's disease have been
tracked to nearby regions of chromosome 6p, Li continued.
Interleukin-23 (IL-23) has a regulatory role in Th1-mediated
inflammation, and drugs that target IL-23 have shown activity in both
psoriasis and Crohn's disease.
In contrast to the genetic and molecular evidence linking IBD and
psoriasis, observational/epidemiologic evidence is minimal, consisting
of a few smaller observational studies. Additionally, IBD and
psoriasis have certain risk factors in common, such as smoking and
obesity.
The relatively modest volume of observational/epidemiologic data
provided a rationale to examine associations between the conditions in
a large cohort.
Li and colleagues analyzed data on 174,646 participants in the Nurses'
Health Study (NHS) and NHS II. Investigators identified participants
who had psoriasis at enrollment and developed IBD during follow-up.
Diagnoses were confirmed by reviewing medical records, self-reports,
and follow-up reports from both studies and reconciling any conflicts.
Investigators identified about 4,400 cases of psoriasis in the two
studies combined. As compared with participants who did not have
psoriasis, those participants with psoriasis were older, had a higher
body mass index, were less active, consumed more alcohol, and were
more likely to be smokers.
During follow-up (1996 to 2008 in NHS and 1991 to 2007 in NHS II), 423
study participants developed Crohn's disease (N=187) or ulcerative
colitis (N=236).
Comparing IBD cases in study participants with and without psoriasis,
Li and colleagues found statistically significant associations for
Crohn's disease but not ulcerative colitis or IBD overall. In fully
adjusted analyses, psoriasis patients had about a four-fold greater
incidence of Crohn's disease:
Relative risk (RR) 4.08 -- NHS
RR 3.81 -- NHS II
RR 3.91 -- Combined
In contrast, psoriasis had an inconsistent association with ulcerative
colitis: a numeric increase in risk in NHS (RR 1.91), a reduced risk
in NHS II (RR 0.68), and a smaller numeric increase in the combined
analysis (RR 1.23). None of the differences achieved statistical
significance.
Study participants who had concomitant psoriasis and psoriatic
arthritis had almost a seven-fold greater likelihood of developing
Crohn's disease during follow-up, Li reported.
A second study from investigators at the same institution provided
more evidence of a link between psoriasis and IBD. Patients seen at an
IBD clinic completed a validated psoriasis screening instrument,
called the Psoriasis Screening Tool (PST), for undiagnosed psoriasis.
Overall, 12.9% of patients met diagnostic criteria for psoriasis.
Unlike the study by Li and colleagues, the prevalence of psoriasis did
not differ between patients with Crohn's disease (13.4%) and those
with ulcerative colitis (12.9%). Most of the patients with IBD had
mild psoriasis that encompassed most of the principal subtypes of the
disease.
The prevalence of psoriasis in the IBD patients was four to six times
higher than the estimated prevalence for the general population, which
ranges between 2% and 3%, Eva Mihalis, and colleagues reported in
their SID poster. "The PST is a useful tool in identifying psoriasis
in a population of patients in an IBD clinic," they concluded.

Ten Minutes Is All It Takes
As part of a new study published in February 2012, researchers at
McMaster University decided to take a closer look at exactly how
massage works to relieve pain and restore damaged muscles. To do this,
they assessed the exercise capacity of 11 men in their early 20s,
after which each subject exercised to exhaustion on a bicycle for a
total workout time of over 70 minutes.
A therapist then applied massage oil to both of each of the subjects’
legs during a brief, 10-minute rest period, performing a massage on
one thigh muscle while leaving the other leg untreated as a control.
Researchers took muscle biopsies from the subjects’ quadriceps before
the exercise, immediately following this 10-minute massage, and then
again two-and-a-half hours later.
The results of this laboratory analysis? In just 10 short minutes,
therapeutic massage helped to reverse over an hour’s worth of exercise-
induced muscle damage, one cell at a time.
Soothe Inflammation and Power Up Your Cells
While the value of massage as a tension-busting, pain-relieving
therapy is well established, this study is the first of its kind to
explore the biochemical mechanisms behind massage’s time-tested
benefits. And, as it turns out, stretching and manipulation signals
more than one mode of healing within your muscles’ cellular
infrastructure.
The McMaster researchers found that massage was able to stifle rises
in NFkappaB, TNF-alpha and interleukin-6 (IL-6)—three cell-signaling
factors that play a major role in the inflammatory cascade that
follows exercise-induced muscle trauma. What’s more, the biopsies also
showed an increase in mitochondrial biogenesis signaling—suggesting
that massage sends messages to the body to create more mitochondria,
the structures that serve as cellular power hubs.1-2
The end result of this chemical activity is a significant reduction in
muscular inflammation, paired with less cellular stress and
revitalized cellular energy, delivering pain relief via the same
biochemical processes you’ll see with many pain medications. Given
this ability, the study authors offer massage as one promising route
to speedier injury recovery—not to mention its potential against other
chronic diseases linked to muscular inflammation, such as arthritis
and muscular dystrophy.
Either way, it looks like a little hands-on medicine is a win-win
strategy where your sore, tired muscles are concerned—and this study
serves up some compelling clinical evidence to prove it.
