Joint model crashing - not sure where the error is

Hi all,

I'm attempting to run a somewhat complicated joint model where the first set of data are binomial with a cloglog link and the second set of data are negative binomial (log link). It initially crashed, so I ran it with verbose=T, but I'm not able to see where the error occurred. I've attached a document with my code for formatting the data as well as the verbose output from the model since for some reason I'm unable to copy-paste it directly in the message here. I need an offset in both models, so perhaps I'm not specifying it correctly? I appreciate any insight you might have here!
