Email template not showing

Hello,

In the "Templates and send conditions" screen, the Email template field is blank (it usually pre-populates with 'Template 1') and clicking the "Next" button doesn't take me to the next screen. Screenshot below. 

Does anyone have an idea about what might be happening?

Thank you so much, Kurt. The case is now closed! It turned out there were double quotes in the header row. I've changed that and now formMule works again.
