Would anyone object to removing support for VS < 2010? Specifically, we
have "our own" stdint.h that is needed by lcm.h (i.e. users might need
our copy to build against LCM if using VS < 2010). I am considering
removing it so we don't have to deal with such possibility, and also so
we aren't using our own copy when not needed.

See also https://github.com/lcm-proj/lcm/issues/139.

Sounds reasonable to me. Would be great to get rid of that stdint.h file.
