"On ____________, 2009, before me personally appeared
________________, known to be the person described in and who executed
the foregoing instrument, and acknowledged that the person executed
the same as the person's free act and deed."
I was asked to translate it and I don't know how to word it to make it
sound formal.
Is there anybody out there who whouldn't mind giving me a hand? I
would appreciate it a lot.

I don't translate into Japanese, but why don't you post the best you can do
as a translation? Someone on this list might help correct your Japanese.

I don't mean to be cold and uncaring, but this is a mailing list for
professional translators, who get paid good money for translating legal
documents such as yours. I am not sure why you expect to have someone
translate this for you for free and on short order it seems. I think the FAQ
says something like this, or at least it used to.
You could certainly hire one of the fine professional English to Japanese
translators on this list to do a nice translation for you. Overall, it will
probably not cost too much, since the text is fairly short. You might also
learn something about professional translation if you are interested in
doing this as a career.
You could also try giving us a good story to see if someone will take pity
on you and do the translation for free.

The HONYAKU list is an exchange forum for professional and other
translators working between Japanese and English, and people
interested in the field. You do not have to understand Japanese to
join, but many messages are written partially or entirely in
Japanese.
Anyone can become a member, but please remember that the list is
designed for people working in the field. Spam will not be permitted,
and continuing off-topic threads is actively discouraged.
On the other hand, if you are interested in translation between
Japanese and English, there are a lot of practicing professionals
here, many of whom are happy to share their suggestions and
knowledge.
We hope you'll join and share your insights with us, too.

This is certainly a quote from the FAQ, but it's interesting that you
skipped the paragraph immediately following the one you quoted:
If you are asking for assistance in translating a simple word or phrase for
free, this is probably not the best place to ask, because the people here do
translation for a living. You would not ask your dentist to fix "just one
tooth" for free; please give us the same courtesy.

This group is essentially a playground for spambots. Take a look and

This group could have limited usefulness with a total of 13 posts in
2009.
However if you have a quick, one tooth problem I do sincerely believe
Proz can be useful(it has been for me).

you will get that impression. However as I have kill-filed most of the
spambot sources so I don't see that junk when I use it.
There is still a small group of us old-fashioned Usenet people who
hang out on SLJ, including a few NSJs. It can stll be a useful
place for people wanting small amounts of EJ translation.

plainly shows that we routinely offer advice and assistance to one
another, perfunctory though it may be by times, asking absolutely
nothing in return. Simply quoting the Honyaku FAQ in response to a
query is about as cold-shoulder as it gets.
Rin, let me advise you. We can and do help out with translating simple
phrases around here, but you need to build up some 'social capital'
first, else none but the symptomatically bored are likely to aid you.
This means post with some regularity, help others as you request help
for yourself, prove that you have made the best good-faith effort to
solve your problems yourself before you do so, and provide us with as
much context as possible to ease the burden on your would-be assistants.
Chemical reactions proceed more easily when you lower the energy of
activation beforehand.
