I have attached the combined file. You can upload this file to ResearchGate in the usual way. I used the program called 'pdfunite' that is easily available on Linux.

Thank you Zbyszek and thank you Bill Page. Bill, I have tried to download the pdfunite program for ubuntu. It downloads as pdfunite.1, and when I try to use it I get an error message. I must be doing something wrong? I am using ubuntu 16.04 LTS operating system. It would be very useful to get it working right as Zbyszek noticed.

Garret,

The pdfunite program is part of the poppler utilities package. This package is often already installed on linux by default. On Ubuntu linux the correct command to install the package is:

    $ sudo apt install poppler-utils
