Mine didn't come with the terminal cable and I purchased it new in 2010. I just assumed it was a 'purchase separately' type deal. The 2.5" and other adapters did come with it, but they're an easy eBay replacement for you..

I'm missing something very basic here and I just have a brain lock and need
    some help.

I got my DDI second hand, and I suspect I didn't get everything that
originally came with it. Does the DDI come with the Seagate terminal cable
and adapters for 2.5 in drives? I suspect this is a 9 pin serial to Seagate
cable and not the USB to Seagate connector since the DDI probably doesn't
    load USB support.

As I don't have this cable where can I purchase a replacement and/or find
    plans to build my own?

Yes it does come with that cable.... You can buy one, just can't recall
    where I ordered mine from
