Questionnaire for BPO Employees

Dear Respondent,
I Ms. Megha K. Juvekar a research scholar under Mumbai University conducting a research on “A study on factors influencing Work Life Balance and Employee Turnover Intentions among the BPO employees in Mumbai” under Mumbai University, request you to fill the following questionnaire. The response given by you will be kept strictly confidential and will be used for research purpose only.

    After work, I come home too tired to do some of the things that I like to do.
    On the job, I have so much of work to do that it takes away my personal interests.
    My family, friends dislike how often I am preoccupied with my work while I am at home.
    My work takes up time that I had like to spend with my family and friends.
    My job or career interferes with my responsibilities at home, such as cooking, repairs, paying bills, childcare.
    My job or career keeps me from spending the amount of time I would like to spend with my family.
    I am too tired at work because of the things I have to do at home.
    My personal demands are great that it takes away from my work.
    My superiors and peers dislike how often I am preoccupied with my personal life at while at work.
    My personal life takes up time that I would like to spend at work.
    My home life interferes with my responsibilities at work, such as getting to work on time, accomplishing daily task or working overtime. 
    My home life keeps me from spending the amount of time I would like to spend on the job or career related activities. 
    I actively look for a new job in the next year.
    I often think about quitting.
    I will probably look for a new job in the next year.
    If I quit my current job, the chances that I would be able to find another job which is as good as, or better than my present one is high.
    I have to leave this job; I would have another job as good as this one within a month.
    There is no doubt in my mind that I can find a job that is at least as good as the one I now have
    Given my age, education, and the general economic condition, the chance of attaining a suitable position in some other organization is slim (reverse-coded)
    The chance of finding another job that would be acceptable is high
    It would be easy to find acceptable alternative employment.
    Sleep disorder
    Digestive system related disorder
    Eye sight problem
    Depression
    Severe stomach related problems
    Ear / hearing problem
    Nervousness
    Voice / speech loss disorder
    Lethargy (Laziness)
    Chest related infection
    Backache
    Overweight / Obesity
    Loss of appetite
    Hypertension (High BP)

Dear Megha, 

In research one should be as careful as Arjuna while piercing bird's eye. I don't think you would find more than 5 respondents here. :(

Sir,

In my survey, there were total 25 questions out of which two were standardised in form. The standardised questions were taken from following researchers. Rest all questions are been framed by me.

1. Work life balance - Gutek et.al (1991) and Frone et.al (1992).

2. Employee Turnover - Cammann.C, Fichman.M, Jenkins. D and Klesh.J (1979) and Mowday et.al, (1984), Billings and Wemmerus (1983), Arnold and Feldman (1982), Michaels and Spector (1982), Peters et.al (1981) and Coverdale and Terborg, (1980).

In case if you are using above two questions you may give the credit to the authors. However you can use my scales.
