The organizations without formal leadership, like The Path, have council gatherings like that. These gatherings cover light to serious topics, like changes to structure or festivals. The most serious issue I've ever seen discussed were murder accusations.
There are protocals for everything.

There are times when things need to be investigated in the Path.
There is a mechanism for doing this, it is called a council of the wise, and any matter pertaining to the coven can be investigagted by or for a council. The one who makes a compaint can have it looked into and a decision is made to what action is appropriate, but ti shild always be remembered that the action recommended can also be in regard to the one complaining if their complaint is found to be malicious or false.
From time to time, Masters outside a coven may take on the investigation of a matter. They will investigate any allegations that are made, but they will also investigate those making them.
At a council, all members of the coven may be heard, and all who are entitled to vote will vote except the one who is the subject of the decision.
