how to enter normal mode by default?

normal mode is the default, unless you have set 'insertmode' option

Does it happen with vim -u NONE -N
Do you have an escape mapping somehow?

Maybe it was fixed by patch 7.4.389.
It could be avoided by putting "set t_RV=" and/or "set t_u7=" in vimrc for workaround.

I meant set empty value to the option (i.e. disable t_RV feature).
