Demo app using questionnaire resource?

Hi. Sorry if this is off topic, but does anybody know of a demo app showing the questionnaire resource? Ideally rendering a form dynamically based on any questionnaire resource, so not hard coded. And then ability to submit it back to the FHIR server... For dstu2.

This may not be strictly related to the group topic, so apologies for that. Not sure where else to ask.

We have this working natively on iOS, will release in the next 2-3 weeks in case that's interesting to you.

Thanks. Yes it'd be of interest. Good if you let me know how it goes, and it'd be useful for me with general comments on how good you think the Questionnaire resource is also. (But possibly out of context here)

In the meantime I've made a web demo app that uses Formly, a Javascript form library, to display the FHIR Questionnnaire. The app takes the FHIR questionnaire and converts it to the formly model, and uses Formly to display it in the browser. Then it takes the answers and put them back into a FHIR QuestionnaireAnswers. It's not very compliant and just a test. The code is here: https://bitbucket.org/ehelse/fhirformly (it's sort of a SMART-on-FHIR compatible web app)

I wanted to have a look at your app that uses FHIR Questionnaire/formly however I cannot access it through bitbucket (You do not have access to this repository). Is the code available somehow?

Hi,

I have various versions, and removed some because of updating of versions etc. It depends a bit if you want to view or upload QuestionnaireResponses. If you want to show forms, I have some other ones also. 

One of the ones for filling in is here: https://bitbucket.org/ehelse/mhealthformapp (a mobile app for filling in questionnaires).

Another is the one you tried, now opened up again: https://bitbucket.org/ehelse/fhirformly/ (I opened it back up)

Both of these are a bit out of date, and also shouldn't really be seen as anything but a rough demo (my coding style is a bit guerilla-like).

Yes sure, but thanks a lot! Going through a demo is hopefully easier then implement a demo for myself ....

For those looking for dynamically rendered Questionnaire resources natively on iOS or Android, we have a framework with sample applications that take FHIR questionnaires and render them using ResearchKit/ResearchStack. Links to the sample apps:


That said, a good online Questionnaire editor would be an AWESOME thing to have!!
