Which QuadStick model has a connected 4-part mouthpiece

Looking for information on an unfamiliar model of QuadStick: it has a fourth hole in the mouthpiece in lieu of a side straw. Any special troubleshooting considerations for this model? We can’t even get it to run through profile selections effectively, much less communicate with the PS4.

I’ve ruled out potential problems with the console, the connecting cables, the firmware version, the configuration profile, etc by putting identical settings on a different QuadStick. My QuadStick FPS is able to successfully boot in PS4 mode with a USB connection to the DS controller. My client's mystery model QuadStick would not work with the same settings and same physical cables.

If there is a customer service line or something I should be calling with this question, feel free to point me there! Not sure what else is left to troubleshoot before resigning ourselves to the idea that this controller might be a dud.

The fourth hole on the mouthpiece IS the side tube.  There is no difference.  Make sure the fourth hole tube is connected to the correct Luer fitting.

If they don't look like this, how do they differ?   When in the video do they look different?  The timing should match within a few seconds.

Send in a debugging report with QMP:
  1. Plug the Quadstick into the PC
  2. Start QMP
  3. Wait 20 seconds
  4. Click in the Transcript pane at the bottom of the window.
  5. Press F1 key
  6. Answer prompt to send in the debugging report
  7. EXIT QMP.  Ignore any error message.
  8. Send me an email so I know to go look for the report.

I sent a debugging report with QMP.

I took a step back to evaluate how the misbehaving QuadStick performs with PC. Mode selection is not reliable there either, but since you have file visibility with the PC I got a better clue as to why: when I upload csv files to this QuadStick, the next time the device starts up the files will either be a) gone entirely or b) reformatted in such a way as to not retain multiple tabs. Hopefully that is useful context for your debugging!

There is definitely something wrong with the configuration files.  They don't appear to have any content, maybe just one line, if that.

If you want to try something before sending it in for repair, forcefully erase the flash then copy the CSV files from your working Quadstick over to the mystery one.

Ah, that doesn't seem to have done it. Files that were copied over disappeared again! Darn. Thank you for directing me to that option, though, I hadn't tried a force erase before.

What is the process to send in for repair?
