Custom Checks Template Code Please?

I edited the custom Checks Template and now I can't fix what I destroyed.. It would be nice if there was a "back to defult" button on the system but for the mean time can anyone cut and paste there original template code, I can't wait to contribute more to this group.

Here's a tip: before you edit a template be sure to 'duplicate' it. Then you always have a back up copy in case something like this happens. Hope that helps!
