Send condition field missing

We are trying to install Formmule in a sheet and when we get to step 2 to set up the email template the send condition box is completely missing. We don't have any empty columns, no duplicate headers, we've cleared cache/cookies but nothing we do is bringing the send condition box back so we can enable the condition for sending the email.

Love your feedback or suggestions. We've used this for years and been super successful and never hit a snag. Thanks ahead for your brain power!

Look at the message in the description of this group.  There is a tool to identify things that can cause Formmule go get stuck in the second step.
