Could you check what view you are currently displaying in your Outline tab? (in case you don’t know, you determine this by looking at the list of views in the left-hand panel and looking for one whose name is in bold red characters).
The behavior you are seeing is what happens when you create a new task while you are in a filtered view, and the task does not pass the filters so it’s dropped from the display. This can be very frustrating if the task _will_ qualify when you are done entering it, for example if you have a view that’s filtered by context and your new task is going to have the “right” context but it’s dropped before you can assign its context.
Many people deal with this by doing most of their task entry in a view that isn’t filtered, such as _All_Tasks_. But it sounds as though you have a filtered view in your Outline tab that requires a context or requires a date.
Sometimes we get views in a tab that don’t really belong there. If that’s what happened to you, I would recommend bringing the All Tasks view back to your outline tab (while in the Outline tab, find “all tasks” in the list on the left and click on it) and then locking it so it doesn’t change again (after you get All Tasks displayed, right-click on the Outline tab and check “set as default for this tab” then right click again and check “lock default in this tab”)
If you really want to have a filtered view for the Outline tab, I would recommend making a new tab (click on the [+] icon after the tabs) making it the first tab (click the new tab and drag it all the way to the left) and then set it to All Tasks and lock it as above. Then use this tab for entering new tasks. The step of dragging a tab, by the way, will not work if you have already locked other tabs, you might need to temporarily unlock them.
FYI the reason I suggest making the new tab come first, is that there are some interesting things you can do with synchronizing selection and zoom among tabs, The synchronization works through the first tab, and if the first tab’s display is missing some tasks the results of the synchronization might not be what you expected. So it’s good to have an All Tasks display in your first tab.

I am struggling to remember what changes were made to solve this
problem - which I think remains a significant issue for new comers
(and indeed for established users) there have been suggestions that
new/amended items should remain on view in a filtered view where they
no longer match the filter criteria with perhaps a special tick box or
something that allows them to be dismissed
