Hi Karen,
I can replicate the problem in both English and in the version with translation. The issue is the same as before – you are using the same entry for “name” within the same list of option (list_name). In effect you are saying for question 1, option A is yes, and option A is no – so this is creating the error you see. See also the image below. There are too many of these instances for me to send a corrected form, but I hope this is clear.

I've been trying to fix this all day.
I upload the english version and it is 100% ok
I add in the translation to both the survey and the choices sheet labels, then all of the single and multi select answers stop working properly
Can anyone help?
