PID.22 Ethnic Group refer to HL70189 table.
In this table there is two coding systems for same values.
When any value use from HL70189 coding system.
"IZ_1_Admin_Child" Test Cases generate errors.
They only accept CDCREC values.
Implementation Guide for Immunization Message refer to this table.
Then why validation tool does not accept HL70189 values.
So which one coding system is valid.

2135-2Hispanic or LatinoCDCREC
2186-5Not Hispanic or LatinoCDCREC
HHispanic or LatinoHL70189
NNot Hispanic or LatinoHL70189
UUnknownHL70189
UNKunknownNULLFL

please note that this is addressed in the addendum to the IG. (http://www.cdc.gov/vaccines/programs/iis/technical-guidance/downloads/hl7-clarification-R4.pdf)

It states:
Ethnicity Group value
set is based upon
CDC Race &
Ethnicity. Next version
of IG would remove
reference to HL7
Table 0189. This does
have HL7 Null flavor
concept which has a
different code system OID.

The addendum that is linked also says the following: (http://www.cdc.gov/vaccines/programs/iis/technical-guidance/downloads/hl7-clarification-R4.pdf)

HL7 table 0189 displayed in the
Implementation Guide refers to two
values sets. The US ethnicity codes are
actually from the CDCREC table. They
should be identified as CDCREC. The
HL7 2.4 codes are the actual values in
HL70189 and should be labeled
accordingly. In addition it is not clear
which one is meant by “reserved for
governmentally assigned codes” in the
implementation guide. Therefore, either
is acceptable. That is, the first triplet of
PID.22 may contain a code from either
value set given in table 0189 or
CDCREC. In addition, the second triplet
may also be valued with a code from
either value set given in table 0189 or
CDCREC. The order in which the codes
appear is not significant. Examples for
Hispanic or Latino include “H^Hispanic
or Latino^HL70189”, “2135-2^Hispanic
or Latino^CDCREC”, “H^Hispanic or
Latino^HL70189^2135-2^Hispanic or
Latino^CDCREC”, and so on.

So PID.22 of the "IZ_1_Admin_Child" Test Case should be able to accept either CDCREC, HL70189, or NULLFL.

You are right, the tool should be updated to accept CDCREC, HL70189, or NULLFL.

Thank you for your comment.

How should ethnic group be sent in the HL7 message for patients who have declined to give their ethnic group, or whose ethnic group is unknown?

Are these acceptable values for PID-22 and NK1-28 (ethnic group)?
  • ^Declined^CDCREC
  • U^Unknown^HL70189

Hi,

Per the immunization subject matter experts (SMEs) who work with NIST, PID-22 (Ethnic Group) should be left empty when a patient declines to give their ethnic group or when the ethnic group is unknown - usage for PID-22 is RE (required, but may be empty). Usage for NK1-28 (Ethnic Group) is Optional, so the SMEs have no official position as to how this field is to be populated; but NK1-28 could be left empty when the next-of-kin declines to give their ethnic group or when the ethnic group is unknown.

Thank you for your question,

Sheryl
