Addition and deletion of partner simultaneously in LLP

Hi

There is LLP with two partners one will resign and new partner will join

Can single form 3 and 4 be filed for addition and deletion

Also will there be two different supplementary agreements or one agreement should be done 

Kindly guide

One agreement can be done and also both addition and resignation can be done in one form 3 and form 4.
