Standard Supervision App

Hello,

I would like the standard supervisory app to be put in my project space so that I can test/use it. On the tutorial site it says:

"The Standard Supervisory App must be already setup in your project space, before you can use it. If it’s not setup, please contact your Dimagi FM with a request to set it up for your project."

How do I submit that request? It isn't real clear on the website who to contact for what or how to contact different people. What does "FM" stand for?
