I am forever forgetting my passwords and having to re-set them. Can anyone recommend a safe and uncomplicated storage system for passwords?

How about Apple’s Keychain?

I used to use 1Password but given the version number changes, subscription model and possibly their new design (not seen it) I’m migrating over to Keychain and using Notes with locked pages for those items that Keychain doesn’t suit.

I use Bitwarden. It's free and runs - and syncs - on my Macs, iPhone and iPad.

I’ve been using 1Password for many a year. I think it’s excellent, enabling me to use long passwords and to paste them by using touch.

I use 1 Passport, which is excellent. But I am thinking of changing as the annual fee I find annoying. Thank you for asking this! Food for thought here also!

I save all my passwords into Safari, the same with 2factor verification codes, so they’re all retrievable easily and will automatically fill into login boxes on a website.

I also use secure Notes for other stuff.

All free, built into macOS and syncs via ICloud between my devices

I used  1Pasword for years, however I stopped when they introduced the subs model, I also use Bitwarden, just as good and cheaper only £10 pa for the premium model.
It works for me

I also use 1Password. I use it to keep all my passwords, logins, credit card details and other information I want to keep secure but handy to refer to when needed. Many websites have a 1Password app to enable quick logins. If you change a password it will ask if you want to save the new one and will also suggest a new secure password (as will Apple Keychain). It is worth the money.
 I wouldn't trust any free password apps; there are warnings about them being free for a reason, mainly to sell your info (see https://askleo.com).
The Apple keychain is helpful, but not as much as 1Password. 

Changing the subject somewhat, another great and secure website is https://www.immobilise.com The national property register website where you can record all your personal items in case they get stolen i.e. mobile phones, computers, cameras, and jewellery etc. etc. . Log their serial numbers, model numbers, receipts etc.

Doubtless too late a twopenneth to be of help, but I was a longtime 1Password user but grew tired that they also moved to subs model (I'd already bought it at full price and wasn't wanting/needing upgrades). 

It's a great app, but using Apple Keychain in Safari (which syncs seamlessly to Mac, iPad, iPhone) plus some FaceID protected Apple Notes for useful things to store (memorable words, PINs, Passport Numbers etc) I really haven't missed it one jot this past two years.

Further, 1Password as I understand it all used to be stored in an encrypted local vault on one's HDD. I believe those days are over and it all hides in the cloud, albeit encrypted. But LastPass were recently hacked (lots of data stolen but again, encrypted) but a shame and a headache.  So I'm sticking with simple and free.

I'm sure all told, just jotted down on back of an envelope hidden in the house is in all probability, probably safest! but Apple Keychain works well and seamlessly, so I'll stay put!
