This bug is more serious than I first thought. It is purely related to the use of VoiceOver with PT. If VO is turned off, the bug is not there any more.
I use PT 2024.3 with last version of Ventura. If I turn off VoiceOver and let an sighted assistant open PT in a session, go to the Setup/i/O dialog, and press the default button in any of the tags Input/Output or Bus, PT will immediately work as expected and stay open in the I/O dialog.
If we turn on VoiceOver and I press the same default button in the same session by using VO-space key, PT will immediately crash, or first show the error message I pasted into my previous message in this thread. The error message contain words like accessibility, so it conforms that this is related to VoiceOver.
However, I did yet another test. I tried to import tracks from a different session by using the option-shift-I command to import session data. If I do this with VoiceOver on, PT will crash immediately when I select the file I would like to import from. If we do the same with VoiceOVer off, everything works as expected and the tracks will be imported. The solution in this case is to use Voiceover to locate the file you would like to import from, then turn off VoiceOver and press return and wait for a few seconds. Then you can turn on VO, select the tracks you would like to import, then turn off VoiceOver again and press return a copule of times during the next minute to complete the import procedure. This works as expected.
But I hope Slau or someone else close to the accessibility team at AVID read this and report the bug to AVID so it can be fixed as soon as possible.
