Remap mouse button 5 + scroll wheel to horizontal scroll

Hi,

I would like button 5 + the scroll wheel to act as horizontal scroll.  In other words, the scroll wheel on its own does the normal vertical scroll, but while holding down button 5 (a thumb button), the scroll wheel scrolls horizontally instead.  Any tips on how to accomplish this?

I've added this funtion at the latest beta version.
Please upgrade Karabiner and use new setting by this private.xml:
