Both AtoM and ICA-AtoM are built in PHP, using the Symfony 1.x framework - so you will need to familiarize yourself with Symfony 1.4 to develop new features or modules:
In general, we recommend that developers adding features to AtoM follow Symfony's plugin development model. Most of the existing modules in AtoM and ICA-AtoM have been built as plugins. Here is an earlier user forum post where we provide links to the Symfony plugin development documentation:

As well, these posts indicate that GAIA Resources, another development company based in Australia, was developing a loans module for AtoM 2. You may wish to try contacting them and seeing if they have finished this yet, and if so, if the code is publicly available:
Note that Kehan Harman of GAIA suggests in the second post that he was developing this loans module by copying and adapting the existing accessions module. This might be a good way for you to proceed as well.
We have also previously heard that XerCode, yet another development company in Galicia, were also working on a loans module. See:
In both cases, I have not yet heard about the outcome of these projects, but you might be able to talk to them. Both Edgar of Xercode and Kehan of Gaia Resources participate in our user forum. Even if they are not able to share the code, they might have suggestions for you!
