Once in a while I have a LilyPond project and Frescobaldi is one of my favourite tools.
I have a timidity server that allows Frescobaldi to play midi.
When inputting notes via the MIDI keyboard I do not hear the sound of the notes played. I seem to recall that I could hear the notes when I did an earlier project with Frescobaldi.

Did I miss a necessary setting?

Yes, probably, as it should work.
I haven't used MIDI input lately, but about 2 years ago I wrote a short
tutorial in the wiki:
https://github.com/frescobaldi/frescobaldi/wiki/MIDI-input-on-Linux

Does it help?

IIUC I need to connect the keyboard to both Frescobaldi and Timidity ? Frescobaldi is (also) connected to Timidity but doesn't pass through MIDI input?

I've just made a test on Fedora 36, Frescobaldi 3.1.3.
If you install fluidsynth and qsynt, then launch qsynth (GUI), the MIDI
keyboard output works immediately.
Then you only have to check the MIDI settings in Frescobaldi. You might
have to reload the MIDI ports and choose the right port for playback
output, but this has nothing to do with MIDI input and keyboard
playback. Keyboard playback must be set up in your system, Frescobaldi
doesn't do anything.

I don't use Timidity.
