Need help designing a custom business search

Hello,

I am a total newbie to this, so excuse me if this is a dumb question.  I need to make a search page like this one http://rxcut.trihopegroup.net/en/PharmacySearch.html customised to a sprcific set of retailers, searchable by address/zip code radius. 

I have been unable to find any how tos or tutorials on this structure.  Any help would be greatly appreciated!
