installation query

Hello All,
   I have installed erpnext on my local machine. while installion i got following error

Executing execute:webnotes.delete_doc('DocType Mapper', 'Delivery Note-Packing Slip') in rorgt
Success
Executing patches.april_2013.p06_default_cost_center in rorgt

patches.april_2013.p06_default_cost_center: failed: STOPPED

i have successfully completed the company setup and refresh the page.  All modules in app page disappear for administrator (are listed in modules menu). i also did not found hide /unhide module link in setup page. Is this new feature?  what should i do to appear all modules to administrator?
