Group,
Today on the call someone briefly brought up the topic of Standing
Orders in the Maternal/Newborn department. I would love to talk more
about "what is a good process" for L&D Admissions and Newborn Nursery
Admission Orders.
Right now our nurses start the order set and write it as a Telephone/
Verbal Order even though we know they don't actually call the
physician. This has caused the physicians confusion since we started
using Order Source and entering in all T/V orders and having them show
up on the physician's sign queue. Mostly because we still require the
physicians to sign the paper copy also because we don't have all
nursing orders built into OE yet.
Let me know if anyone has a great solution to this one.

Try creating a standing order - order source for this purpose that does
not queue for signature since the physician is still signing the paper
copy. Then, once all your orders are built in OE and there is no need
for the paper copy, switch it to queue for signature.

This method will work but can cause a lot of orders to end up in the providers queue which can be a big dissatisfier when starting cpoe.  What we did is create one order "Newborn Protocol" that is entered with a source called "Sign Protocol" which goes to queue for signature, which allows the entire newborn order set to then be entered with a "protocol' order source that does not go to queue for signature. This satisfies the need for protocols to be initiated by a provider,  allows nursing to use the protocol, and reduces the number of orders in the providers queue.  Realize though that this method requires that a protocol be written in policy and approved by med exec..

How would you meet meaningful use this way? The MD is not entering the medications.

We have done the same concept the protocol is in a cds then the meds are all prn per protocol so the dr has actually ordered the med I can send you a screen shot if you want it

I hope that this is what you are looking for. I hope that I didn't miss understand you all.  this is what we are doing and we got this idea from lake Regional in Osage beach mo.  they call their's "guide" orders but I really like the Sign Protocol "  the enduser would understand MUCH better.  we ran the guide by the dr's and they looked at me like "" WHAT"  I don't know you all so that is the G rated version of what they really were saying.

I believe that as long as the regulations related to protocol orders are followed that because it is initiated by the provider these orders were truly ordered by the provider and therefore count for both the denominator and numerator, while non emergent verbal orders would count toward the denominator and NOT the numerator which then hurts your numbers.  I hope I said that correctly.

Alexa and anyone else who could help out!

We are just getting started on CPOE. We are in the process of building our orders and plan to have our providers up live by November 1. We will need to start training our nursing staff (including unit secretaries) on how to enter orders from POM vs OE. We would like to have them as resources for our providers as they start out. Does anyone have any type of training material that could help us out? (for providers or nursing)

We are a 5.65 Client Server Site.
