Problems with ADO Sample from InfoPath SDK

Hi... I'm havnig trouble with the ADO_CON sample form and database that is provided in the InfoPath 2003 SDK. I can't get it to work. For now, I have everything on one machine -- keep it simple to start. I have Published the provided sample form to a shared folder on the same machine -- I simply created an additional folder next to the ADO_CON folder that comes with the SDK and tried to publish there.

When I try to open this form in InfoPath, I get a dialog box called
"Please Enter MS JET OLE DB Initialization Information"

Why does this come up and what do I need to provide?
It has infnwind.mdb as the DataSource -- that seems right, since that is the MDB file.
It has Admin as the User Name -- why?
It has Password blank, Provider String blank, and Open Mode is set to DB_MODE_READWRITE as the default from a drop-down of choices.

I have done a fair amount of development over the years with ASP Classic and now ASP.NET, including working with .MDB files, connection strings, and so on. But this has me stumped, because the documentation in the SDK doesn't mention this dialog box at all.

I don't understand why this box appears and what I should do? Please help if you can!
