Originally in TOS, each starship had its own insignia, yes. The Enterprise's
was the arrowhead-shaped one that we've come to know and love. This was
more or less carried on as the series progressed, though some fans have
interpreted the different symbols to indicate higher rank (e.g., Commodore).
Note that the idea that "the Enterprise was the only ship to make it back to
Earth after its five year mission" is non-canon material, not supported (to
my knowledge) in any of the video stories. (For that matter, the idea that
other starships also went on five-year missions is also non-canon. The
Enterprise was at least 13 years old by the beginning of Star Trek, and it
seems unlikely, to me, that Starfleet would just suddenly - relatively
speaking - decide to send all its starships on "five year missions".)

In ST:TOS, each of the 12 or 13 or whatever of the Constitution Class ships
(like the Enterprise) had it's own crest. The Enterprise's was the arrowhead,
Other ships had rectangles, knots, circles, etc.
The arrowhead's never been a symbol of the Federation. The Federation
symbol is the starfield map with laurel leaves or humanoid faces on either
side, and the letters UFP below. As of ST:TMP, the arrowhead was the symbol
of Starfleet. During ST:TOS, I think the left-broket type symbol (like the
one at the front end of the red racing stripe on the side of the Enterprise's
sausage section) was the Starfleet symbol, but I'm not sure... Though I seem
to remember it often showing up behind people calling in from HQ...

True enough, but because they do use the Enterprise Arrowhead as the
Federation symbol, implies that at least the preface to the ST:TMP novelazation
is canon, which if anything from the book should not be canon, the preface
is at the top of the list!
Lesse if I can explain this... There were 13 ships (lucky #) sent out by
Starfleet on 5 year missions. 12 were lost, destroyed, or sold to make a
fast buck. Only the Enterprise (NCC-1701) returned. The Enterprise then
underwent a total refit to bring it up do date because there had been new
technology but whenever the Enterprise stopped in port, they just fixed
problems...no updates. So when the Enterprise returned, they wanted to put
new warp engines on...then they found they had to add stronger nacilles then
they had to redo the computer, then they had to add a bowling alley (by
popular request), then they needed bigger doors for Scotty, so the entire
ship was overhauled in 15 years. While Scotty was turning bolts, the crew
(including Scott) got promoted (after the book _Star_Trek:_The_Lost_Years_)
and in thanks, Starfleet adopted the Enterprise's Arrowhead (or WHATEVER) as
it's symbol. Before that, each ship had their own symbols...now this made
it easier on the writers so they didn't have to think up a new symbol each
time another ship came along. Then came the movies... blah blah blah...
then the Next Generation used the same symbol but on an elliptical
background. In all the movies and TNG, the background behind the Arrow is
the same shape as the saucer section of the Enterprise (Flag Ship of
Starfleet).

In ST:TOS, there were only 12 Enterprise type starships in the
entire fleet. The confusion on starships arose from the
fact that nobody was keeping track of starship names during
the first season, and 12 names in addition to the Enterprise
were aired. In the writers guide (commencing second season)
it was clearly stated that only 12 ships existed, and careful
record was kept of where they were, and who had been destroyed
from one episode to the next.
In the episode (I forget the name) where the Enterprise is accidentally
hurled back to the 20th Century, Kirk actually tells the pilot
of an Air Force jet (accidentally crushed in their tractor beam) that
there are only 12 ships of that class in the fleet.

Ships of a particular class is quite different from ships in the fleet.
The Enterprise (D) is a Galaxy class starship, but there are also other
classes, such as Constellation class.

Er, re-read the first sentence and last sentence of the rivero included
text. They make perfect sense, but you seem to have missed them.
