I can't reconcile everything you describe with what I see, but I guess if you
can't see it, the problem might be the way you're remembering. So...
The closest I can come to what you describe is clicking the button "Page
Number", then selecting an entry such as "Top of Page". And then you say you're
aren't seeing the Built-in selections, just the menu entry at the bottom.
This probably means that your building blocks template has a problem. If it were
missing, Word should generate a new one. Try searching Windows for a files
named: Building Blocks.dotx
Do you turn anything up?

You've found Building Blocks.dotx - that's good. In what file path? Does it resemble
this path
Application Data\Microsoft\Document Building Blocks\1031
Most importantly, is it in a folder with number, and which number?
If you move it out of this folder (move it to My Documents, for example), then start
Word, this should force Word to create a new "clean" copy with the defaults. Does
that make any difference?
I have the same problem, but when I moved BuildingBlocks.docx out of my 1033
directory and restarted Word, the file did not regenerate itself into the
1033 folder. Is there something special you have to do to get Word to
regenerate that file?

there are a couple of ways to fix this problem, as I was able to fix
this problem myself using these two methods.
Method #1: (what I did)
You will need two explorer windows open.
First, go to this folder:
C:\Program Files\Microsoft Office\Office12\Document Parts\1033\
You will find Building Blocks.docx, Do NOTHING to this at this point,
but keep the window open. This one is the default.
Secondly. (Since Windows Vista changed the directory where files are
stored, I will list both directory for both version of windows.)
Windows Vista users:
Go to this directory:
C:\Users\(username)\AppData\Roaming\Microsoft\Document Building
Blocks\1033
XP Users: since I no longer run XP, I can't EXACTLY remember but it
SHOULD be this:
C:\Documents and Settings\(your user name)\Application
Data\Microsoft\Document Building Blocks\1033
It should be SIMILAR to this, but that is where the file should be.
Make sure you are able to view hidden and system files, or you won't be
able to find the directory mentioned above as they are hidden by
windows.
COPY the DEFAULT Building Blocks.docx to your application data
directory and replace that building blocks.docx file with the default.
Restart word and you should be home free.
Method #2:
From what others are saying, Just deleting the Building Blocks.docx
from the application data directory (NOT THE PROGRAM FILES), might fix
the problem, but I Can't confirm that.
