MoJ recommmends candidates declare their interests

It says that the MoJ recommends that candidates declare their
interests before they are MPs, although it is not obligatory.
Perhaps it is a hint for us to gather the declarations!

Quote the exact new rules from the register of member's interests, and
make it clear that if you are elected you will be expected to fill
these details in.
This is necessary for democratic accountability and information about
conflicts of interest. It is clearly immoral to declare these
conflicts to the public only after being elected, isn't it?

13. Members of Parliament are required to complete a registration form
and submit it to the Commissioner within one month of their election
to the House (whether at a general election or a by-election). After
the initial publication of the Register (or, in the case of Members
returned at by-elections, after their initial registration) it is the
responsibility of Members to notify changes in their registrable
interests within four weeks of each change occurring.
The simple mission is to obtain a copy of this form, which is not
immediately obvious to me by searching the internet.
Can anyone get a copy? I imagine it's something similar to one of
those tax forms, with carefully worded advice on the facing page.
