How do I set mandatory fields (both default and custom) for each
ticket type?

Select fields become mandatory when you don't allow the empty option (so
the list of possible options does not start with a | in trac .ini').
Otherwise you can not make fields mandatory without additional plugins
(though there is a dedicated interface for that…).

A. If I don't allow the empty option, then the first option in the
list is the default one? Right? If so, this is a default parameter
mechanism and not mandatory field, right?
B. Are there any working plugins for mandatory fields?

You can select which is the default option in the respective field.

I removed the spaces and the default value, and although the system
selects default parameter (the first on the list).
I'd like the field to be empty, but still mandatory. I don't want the
system to choose the field value for me?

Any suggestions?

So you would want the drop down to have a preselected option like
"Click to Select Option" and then people need to click and choose before
they can click "Create Ticket"? This is not possible in Agilo right now.
Some dialogs (like task creator in the white board) intentionally limit
the fields you can enter, and select empty/default option for a new
task.
What is the problem that you are trying to solve? Does it help to make
the field optional and then have a report for tickets where the field is
unset? That way, you can identify tickets where people missed it and you
can educate them to select ticket properties with more care if need be.

I need to make sure some fields are filled.
As you suggested, I've added a default value: --- for each mandatory
field.
I go over the tickets where this field equals --- and educate my team.
This is not the best solution, but works.
I'd expect a mandatory field...
