You're mixing up x / y coordinates, and this QR code is flipped over, rendering it invalid.

You can use MatrixToImageWriter to render these things rather than debug your own implementation.

Thank you very much for your help. I was able to untangle my mess and also use MatrixToImageWriter. Should I post my entire servlet somewhere?
