Creating An Item Template

I had a designed Item Template active in my Tasks Database for years.  It apparently disappeared in the last month or so.

I tried to recreate the Template but to no avail.   I followed (or at least think I did) the directions in Help.

My Template appears in the System Folder.   However, I cannot get it to active when creating a New Item in the database.

Any suggestions?
