As the original receipt/invoice is needed, could you let us know who was the person shared the scan copy of CDPG from MGKT?

Dear Mr Teoh / Pang / David,

Further to QT12 inspection with AIS today for ESC2 and IVS3, please find the following documents required for the QT12 submission (claim BG).

1) Consultant Cover Letter
2) Supporting letter from MGKT - application letter to claim BG - provided by MGKT
3) Site Photos report (before and after QT12 inspection) (based on AIS's comments)
4) Authorization letter
5) QT12 attendance List
6) Checklist for Defect Liability Period of Water Supply System
7) Original CDPG Invoice - provided by MGKT
8) Bank statement which shown the account number, account's name, bank name. Please hide the confidential information - AIS will transfer to this account

Item 2 and item 7 will be provided by MGKT.

For info, the QT12 application need to submit to AIS 1 month before the last date of DLP. Please find attached the table show the status of QT12 submission your perusal.

We also attached the handover letter from AIS (together with QT11 form) and CDPG invoice for DDC3 and DDC7 for your reference.
