Please been informed that the only submitting letter is should be from client. The consultant cover letter is not acceptable by AIS.

All the documents is for shaft that completed the QT12 inspection (ESC1, ESC2, ESC3, IVS3 & DEWP) :-
i) Cover letter from client - attached the draft for your further action.
ii) SURAT AKUAN KEHILANGAN SALINAN ASAL RESIT (must attached the copy of CDPG payment receipt - attached in this email) - need to fill up by client and cop&sign
iii) EFT form - must attach with the bank statement

For your info, some of the shaft may not have QT12 attendance List and Checklist for Defect Liability Period of Water Supply System which are already taken by Air Selangor.

For ESC1 and ESC3, site photo report is not required because no comments from the officer.

Please find attached the acknowledge copy for submitted QT12 for ESC1, ESC2, ESC3, IVS3 & DEWP for your perusal.

But, after submitted the documents, the officer called and informed that the bank statement should from MRTC, not MMC Gamuda because it is must tally with CDPG payment receipt. The below sample CDPG receipt for IVS3 for your reference.

Kindly assist to forward the MRTC bank statement in order for us to resubmit to Air Selangor soonest possible.

Dear all,

For info, the revised EFT form submitted to En Hanafi of AIS (swapping the document) on 13/11/2024.

He informed that the process of releasing the CDPG to MRTC account may take 30 days (the fastest period).

Please note that there is no official letter from AIS, but MRTC will get the email notification from bank on the release payment.
