Boxes unchecked still field is mandatory

I've installed the official ZAP plugin & even after going through a lot of videos I still can't figure out the exact cause of why it's asking me to fill all the fields even though they're unchecked. If anyone has faced similar issues & solved them, I request you to kindly help me solve mine.

It is worth noting that the ZAP Jenkins plugin is no longer supported, although we will do our best to help with advice and guidance.
