Error uploading xls to xform

I'm uploading my xls form to an xform on the website http://xform-prod.mvpafrica.org/,
and this error message comes up.

For some reason this choice isn't associated with a list.", {u'type':
u'province', u'name': u'bam', u'label': u'Bam'})

My xls forms look like this:

survey

type name label

select one from province province Sélectionnez la province


choices

type name label

province bam Bam
province sanmatenga Sanmatenga

Do you have any ideas what could be causing the error?

Hi emily,

First, please use http://formhub.org -- we're deprecating http://xform-prod.mvpafrica.org/.

Secondly, the choices tab uses the headers:

list name, name, and label, rather than type, name, and label (see http://formhub.org/syntax/)

Attached is emily.xls, which replicates your survey below, and works without errors for me on formhub.

And finally, it might be a good idea to name your list "provinces" rather than "province," so that there isn't name confusion between list name and field name, but thats up to you.
