Dear All,

Here i am getting some error while publishing the xls sheet. Please let me know what to do?. Please find attached xls sheet

Hi all,
We fixed the issue. The problem was:

1. missing the first row in the choices tab
2. having 1, 2, 3 etc. in the excel file as the option display name for the file.

Looks like formhub / xls2xform doesn't currently support that.
