I have added a new MSSQL Output.XSL file that adds:

# Support of named PRIMARY, UNIQUE and FULLTEXT keys
# INDEX support (based on MySQL Output.XSL file)
# FOREIGN KEY support (based on MySQL Output.XSL file)

Please let me know if there are any problems / changes needed.
