better PostgreSQL output and datatypes

Hi all

1st, kudos to the developers for this simple but highly useful tool.

2nd, I have a small patch which changes some stuff in postgresql xslt

- output AUTOINCREMENT columns as SERIAL pseudotype
- do not output DEFAULT NULL or DEFAULT 'NULL', because the former is
a no-op and the latter is always a mistake.
- output proper COMMENT ON statements.


there is also a small change in postgres datatypes.xml , I have fixed
two typos in timezone types.


I hope that this patch will be included in the svn head.
