Displaying "Identify" information

For my BDUG Project - Map 11 - A.2.b.viii – Beauplan’s “A Description of Ukraine" (keyword BDUG), I have uploaded a layer named "Places".  When I click on Identify and then click on a place, a box opens up and shows all of attribute headers from my shape file, but doesn't show the values of the attributes.  What have I done wrong?
