Hi

I put some GDM supply forms in the sono unit.  If you find a newly diagnosed GDM when you are in sono unit, you can fill out this form, order nutritionist and send her to the clinic for supplies, teaching, nutritionist and hrc appt.  I call up and speak to the RN in charge and let her know im sending her up.  that way everything can get started asap.  

there are also quad recalculation forms, myrna can fax them if you fill it out.

does anyone know of a better bc form in spanish and english that we can give in sono unit?

The Female Patient - online version has thousands of patient educational information sheets provided in English and Spanish

The birth control one is in the 2009 July Issue  - not the best info but OK, easy

Another idea, ARHP - again online access to multiple patient education materials

Of course Planned Parenthood has great info as well

Womenshealth.gov has great info - no pictures, however, nicely put together fact sheet

I have a bunch in my folder on the shared drive.

 

Go to:

-Ob Admins Share Drive >> Providers Individual Folders >> Midwives >> Dalke >> Handouts >> Contraception

 

and you'll see a bunch in there. The ones titled "Options" I think are good (in English/Spanish/Creole)
