We have developed a budget for each Fistula patient , 
The operation and treatment is free here , it's a great hospital established for fistula only for women.
The related very important cost is of , travel of the patient , one attendant, family and community motivation to agree for travel , operation, travel of our midwives arranging the travel and preparation for the patient , good nutritional food, cost of healing ,   Counseling and community education activation against girls and women's child marriages , Mis match marriages, safe child birth practices, reaching out for help and treatment. ESSENTIAL ROLE OF LOVING CARE, 
All these activities together has coasted Rs. 30.000, per patient, with out over head cost of IDSP's staff time , my time. 

Kaukab if you look at it , the program is emerging in following direction.
Major steps, 
1. Identify and empowering a community based young woman in high quality medical , nutritional,health conscious activist as a midwife .

2. Launch a community education and development  for concepts and practices of meaning of a child and a girl child, a woman hood, status of a wife, motherhood, how a woman effects a family development processes , 

3. Identify cases of women suffering from fistula , make it a family and community concern, breaking the silent sufferings , and addressing the issue up front and relating the causes to child marriages , sexual abuses to a underage girl by her husband, lack of safe motherhood practices , connecting to medical help services, and thus demonstrating the impact of treatment of fistula on the woman's life. The before and after effect elaborated on the family and community.

4. Finally create a community medial aid and education support centers named MAA will be established by each midwife . 

We plan to establish minimum of ten centers for MAA, in next two years.

Above is the core concept with major actions to create PROGRAM OF MAA, 

I look forward to your suggestions and support and any more advice.

In last two and a half years we have trained 100 or more women as midwives , through them we are learning and Taking each step at a time.

I will speak to my Gynaecologist friends here in U.K and try to see if we can do a short project for a junior here in U.K as these conditions do not exist in U.K, and may be we can get this done free for these girls by an excellent trainee from U.K who is willing to travel to Pk.

Also on average how much does it cost to treat these women?

Your commitment to the miserable conditions of the children identified by ASIF the flim maker is commendable.

If you want more information you can send us a simple questions that a non medical village person can collect for further knowledge and improvement in the living and social practices in the village communities.

At a different note , our midwives are identifying Fistullha cases in rural Bolochistan.

We got two to Karachi and got them operated and treated , the women who suffered from minimum 6 years to max 11 years silently and with shame, our midwives work day and night and finally the women were operated, well fed, rested , got it all cleaned up , we celebrated their new respectable life they dressed in formal new dress and left with their families happy and proud to their villages.

The families got a detail session on the causes of Fistulla. 

The men promised to educate the community on causes of fistula in women .

Main causes.

Child marriages

A child girl is married to father aged man, 

The local Dais messing up the baby delivery cases, 

No trained medical help and support

No community consciousness on wrong practices of above causes.

Now our midwives has identified ten more cases of very young women who were inhumanly raped by their husbands on their wedding night, or has fistulla due to bad child birth , child marriages and pregnancies .

Our midwives are now becoming community activists in reproductive health care .

We have now created a system connecting these suffering women from Balochistan with Karachi 's specialized hospitals for the treatment and cure of Fistula.

We need support to continue this very much needed service to the women of Balochistan.

Please let us know if UK MEDICAL AID CAN HELP.

Ok my colleague paed dermatologist provisionally diagnosed a genetic disorder possibly due to cousins marriages or inbreeding called Xeroderma. She will be discussing in a multidisciplinary meeting and will seek further advice. Essentially these children need to avoid sunlight and need Calcium and Vit D.

They may need other medication which may not be available in Pk. According to her these children are high risk for cancers.

Perhaps there is not much one can do them except to keep them comfortable but their families need to be educated against cousins marriages.

It is a very rare and sad  condition with no cure.

Pl can we have the statistics.

Asif is in coordination with effected children's families. Some families are currently here in Quetta due to the hot weather of Sibi they migrate from Sibi to Quetta for two to three months. Asif is mobilizing these families. After eid Asif will depart for Sibi to mobilize the rest of families to make them agree for their children's treatment in Karachi. 

We are busy to collect the exect number of effected children.

Pl do whatever is necessary to get these children treated I Karachi by a good dermatologist.
I am waiting for the opinion of paediatric dermatologist in UK.
I know a few dermatologist in Karachi and Hyderabad I will get their numbers and will send them to you.
However if you know someone yourself then pl book an appointment.
We are all very disturbed at these pictures, are these children all from one family?

Our initial efforts in last two days has given the following outcome

1. There is an increasing number of incidences of doctors being kidnapped for ransom in SIBI Area. Doctors are reluctant to travel to the area.

 

2. Second option is to get two children as a sample to Karachi IDSP hostel , and get the good quality medical examination and share the report with all concern for future line of action.

 

3. Mr. ASIF the flim maker who was in Quetta for Eid and Ramzan is now going back to SIBI villages and will start motivational work with the families and communities to agree them for Karachi travel.

 

4. It is expected that one or two family members along with ASIF will travel with the children , in a hired vehicle to Karachi immediately after Eid.

My suggestion is that you allow us to use the remaining funds for the purpose you have mentioned .

After the required work is done we send you all the invoices and receipts along with diagnoses results. 

This will save the time and we can carry out the work instantly while we remain within the remaining funds.

Right now we are not sure which doctor to select and how to arrange his or her travel to SIBI and in the remoteness of the villages
