The Wisconsin Division of Mental Health and Substance Abuse Services (DMHSAS) is working on creating program brochures, program webpages, and other communications regarding Community Support Programs (CSP), Comprehensive Community Services (CCS), Community Recovery Services (CRS), and Individual Placement and Support (IPS) to help inform people about these programs. We would like to include quotes from people who feel they have benefitted from being in any of these programs into our communications.

If are you currently are in a program or have been in one of these programs within the last five years, please tell us: 1) Which program you are/were in, 2) In a brief paragraph, how the program has assisted you in your recovery, and 3) Whether you would like to use your full name, first name only, or for it to be anonymous.
