Bulk upload deployment failed: please help

Sorry to bother everyone, but I am trying to make sure I do this right and avoid the PITA of manually creating each deployment/location combo.  I have a .csv file I will attach.  I think I have the fields correct, the new error message just says my upload is in an unknown state.  Is there something I am doing wrong?  I am just really frustrated :)

Leah: Hah 😊.  Maybe next year, I ended up doing all 30 one at a time…. But it’s done.  Yay!
