Publish PDF library version 2.2.40 released

It has been 6 months since the last release of the Publish PDF library. The main change is to fix calls to htmlspecialchars to include the document encoding. Full release notes are at http://www.wikipublisher.org/wiki/ReleaseLog/20121111. Thanks to Vladimir Kulev for the fix to image markup in lists and other nested elements.
