My name is Helen Malith and I am the Commonwealth Psychosocial Support (CPS) Program Lead at Chorus for Country WA.

I am reaching out to share some exciting news. As of October 1, 2024, Chorus will be expanding the CPS Program in the South-West region. This significant initiative is designed to provide crucial support to individuals aged 18 to 65 with severe and persistent mental health challenges, offering a ray of hope to those not on NDIS and experiencing emotional and psychological difficulties. We are thrilled to be extending our services to the Shire of Dardanup, Shire of Harvey, Shire of Donnybrook-Balingup, Shire of Capel, City of Busselton, Shire of Collie and Shire of Augusta and Margaret River within the South-West region.

 

Furthermore, as of October 1, 2024, Forrest Personnel, our hosting agency, will be providing the CPS Program in the Shire of Bridgetown-Greenbushes, Shire of Boyup Brook, Shire of Nannup and Shire of Manjimup within the South-West region.

 

The CPS Program provides holistic support for individuals with mental health challenges. It offers tailored support, focusing on capacity building, enhancing mental well-being, and providing practical assistance to improve the quality of life. Our holistic approach ensures that all aspects of a person's well-being are addressed where necessary, building their confidence and resilience.

Our services are designed to cater to a diverse range of needs, including:

• Personalised Support: One-on-one support with a mental health support worker to address specific concerns and develop effective skills.
• Group: Support groups that promote community connections and shared experiences within the City of Bunbury.
• Resource Navigation: Assistance in accessing additional support services and resources within the community.
• NDIS Eligibility Testing: Assistance with accessing NDIS to help customers meet their support needs and build capacity to improve their overall health and well-being.
