I'd like to invite you to the next Warren Blackwood Human Services Network Meeting; 

 

Thursday 26th September

3.00 pm to 5.00 pm 

Andre Foulon Room, Manjimup Family Centre, 13 Mount St, Manjimup. 

 

Rae de Rusett, of GP Down South will address the meeting.

 

Rae will speak about the My Health Partnerships program, an Innovative Model of Care funded pilot project, being implemented by General Practitioners within the Warren Blackwood region.  My Health Partnerships aims to enhance collaboration and increase access to health and wellbeing services for people across our region. People attending the meeting will also have the opportunity to participate in a ‘round robin’ discussion on local issues.

 

We have varied our usual time to give people who are not available in the morning the opportunity to attend.
